Nephrotic Syndrome, Glomerulonephritis & Ultrasound-Guided Renal Biopsy
Clinical Practice & Guidelines • Dr. Fayaz Ahmad Wani (MD SKIMS, DM Nephrology Osmania Gold Medalist)
1. Understanding Glomerular Kidney Disorders
The glomeruli are the microscopic filtering units of the kidneys. When immune complexes, autoimmune antibodies, or genetic abnormalities inflame or damage the glomerular capillary filtration barrier, patients present with either Nephrotic Syndrome or Nephritic Syndrome (Glomerulonephritis).
With specialized expertise in clinical immunology and renal pathology, Dr. Fayaz Ahmad Wani conducts thorough diagnostic workups, personally performs ultrasound-guided percutaneous renal biopsies, and designs targeted immunosuppressive regimens tailored to histopathological findings.
2. Nephrotic vs Nephritic Presentation
| Clinical Parameter | Nephrotic Syndrome | Nephritic Syndrome (Glomerulonephritis) |
|---|---|---|
| Proteinuria | Heavy (> 3.5 g / 24 hours) | Variable (usually < 2–3 g / 24 hours) |
| Hematuria | Microscopic or absent | Prominent (cola-colored urine, dysmorphic RBCs, RBC casts) |
| Serum Albumin | Severe Hypoalbuminemia (< 3.0 g/dL) | Normal or mildly reduced |
| Edema | Generalized severe edema, periorbital puffiness, anasarca | Mild to moderate periorbital and pedal edema |
| Blood Pressure & Renal Function | Often normal early; hyperlipidemia present | Hypertension and oliguria with rapid rise in creatinine |
3. Conditions Diagnosed & Treated
- Minimal Change Disease (MCD): Highly steroid-responsive nephrotic syndrome common in children and younger adults.
- Focal Segmental Glomerulosclerosis (FSGS): Sclerotic scarring in segments of glomeruli, requiring targeted immunosuppression or calcineurin inhibitors.
- Membranous Nephropathy (MN): Autoantibody-mediated disease (anti-PLA2R screening) requiring immunological risk stratification.
- IgA Nephropathy (Berger's Disease): Most common primary glomerulonephritis globally, presenting with recurrent hematuria following upper respiratory infections.
- Lupus Nephritis: Severe renal involvement of Systemic Lupus Erythematosus (SLE), categorized into ISN/RPS Classes I through VI.
- Rapidly Progressive Glomerulonephritis (RPGN) / ANCA Vasculitis: Medical emergencies necessitating urgent immunosuppressive induction (pulse steroids, cyclophosphamide, rituximab, plasma exchange) to prevent irreversible crescentic scarring.
4. Ultrasound-Guided Percutaneous Renal Biopsy
In adult nephrotic syndrome and progressive glomerulonephritis, an accurate renal tissue diagnosis is indispensable. Dr. Fayaz Ahmad Wani performs percutaneous real-time ultrasound-guided kidney biopsy under local anesthesia, ensuring maximal tissue yield and minimal vascular complication risk. Biopsy specimens are examined via light microscopy, immunofluorescence, and electron microscopy to establish the exact pathological classification before commencing immunosuppressive therapy.
